Our Assessment Process
A Safety Assessment and Functional Assessment are two types of assessments that are often conducted to help seniors age in place, which means to live safely and comfortably in their own homes or communities as they grow older.
A Safety Assessment is designed to identify potential hazards or safety risks that may exist in the senior's living environment. This type of assessment typically involves an evaluation of the home's physical environment, such as lighting, stairs, and bathroom accessibility, as well the senior's ability to manage daily living tasks safely and efficiently. The goal of a safety assessment is to identify and address any safety risks that may put the senior at risk of falls, accidents, or other related injuries.
On the other hand, a Functional Assessment is designed to evaluate the senior's ability to perform daily living tasks independently, such as bathing, dressing, grooming, cooking, and cleaning. This type of assessment typically involves an evaluation of the senior's physical and cognitive abilities, as well as their overall health and well-being. The goal of a functional assessment is to identify any areas where the senior may need assistance or support in order to maintain their independence and quality of life at home.

Our @Home Plan provides the strategy and course of action needed to adjust your home to your current needs to ensure it is the safest environment for you or a loved one to properly begin aging in place. It is flexible and open, allowing for changes as you or your loved one requires them over time.
We offer a 235-point Safety Assessment along with a 120-point Functional Assessment will provide the information needed to create the Home for Life! @Home Plan. This plan will include:
- Client Goals – Current and future.
- Budget – Immediate and ongoing.
- Assessment – Summary of Safety and Functional assessments.
- Recommendations – Based on Comfort to Critical color matrix using Green, Yellow, and Red.
- Implementation Plan / Timelines - In line with budget and ongoing.
This process usually takes 2 to 3 hours to complete at the home so you can begin to formulate setting up your home for success so aging in place can become a reality for you or your loved one. A @Home Plan is then created and presented in person within a day or two. We will spend the time necessary to review each section of the plan with our clients and answer questions to help you feel at ease and support you during this difficult time!
What is the cost?
It depends on the size of the home, time spent during the assessments and preparation of the plan. A Home for Life! @Home Plan generally costs from $375 to $475.
